I need transport to Addenbrooke's Hospital on 8th June arriving at the main entrance at 11:50am for a ct scan after a recent heart attack. I am unable to drive for 4 weeks and have to get to hospital for the scan. I would also need a receipt from the driver to get reimbursement from the CUH Trust. Thank you Mark
