Personal care in a stimulating environment. Wears hearing aid so has hearing issues. Suffered stroke some four months ago and is left with right sided weakness. Has macular degeneration and wears glasses. Stoma bag in situ. Continent but needs assistance to transfer to toilet, bed and chair. Is privately funded. Prefers plain food nothing foreign or with sauces only gravy. Is mentally alert no evidence of dementia. Needs assessment available on request.
