Dear Sir / Madam. My name is Nadeem Talib and I live in Rochdale. I need a private medical assessment followed by a medical report regarding these issues; 1. I have some injury scars, I have some x-ray reports, medical reports and pictures in relation to them. I need these to be assessed by a relevant medical professional. 2. I am also suffering from post-traumatic stress, I also need these to be assessed. 3. Assessment of overall physical and mental situation and whether, I am mentally affected by this stress. Could you please guide me that can I get these assessed by you or your organisation and what will be the charges for that. If it’s possible, could you please give me first short appointment (one-one appointment with relevant professional), so that I can discuss all above issues in person.
