When I came from India I have my Christian name fowelled by Singh.When I get my nationality my solicitor used my surname Jandu which is used on bank a/c ,driving licence, utility bill my voting list etc etc etc.Now I have applied for O.C I card to Indian embassy so I need effidevit to show that I am the same person attested by notary public.that is why I want an appointment for his signature.
