Hi My name is Van hoang vo my date of birth is 10-08-1969 a bout 30 ago. Disabled wheelchair service centre provided a wheelchair. But now it was damaged I can not use it now I am a disabled I have got right leg polio. I got only one leg I use crutches to walk but sometimes I have got spasm on to my leg. Can you please check and provide a wheelchair. Many thanks
