    I have T8 complete paralysis and have dislocated and broken my shoulder in the recent past. I require a letter of recommendation from a physiotherapist to support an application of funding for a much needed wheelchair through the DTD trust. It should be a comprehensive statement on the appropriate agency's headed notepaper describing the my circumstances. It should outline why the application is being submitted and the benefits it will bring if successful. It needs to be specific to the item for which funding is sought. It also needs to confirm that the item (wheelchair) is safe, appropriate and meets my needs. The wheelchair I require needs to be lightweight and specialist so that I can lift it in and out of a vehicle. It also needs to be appropriate for outdoor use, as my current wheelchair is not. The one I am looking to get is 4kg, as opposed to a standard 12kg. I require an appointment for the supporting letter of recommendation.Read more
