Medical report to support NHS ill health retirement application appeal. I need it to confirm that Itis likely I will remain unfit for my very busy role as a community psychiatric nurse on a permanent basis ,even after surgery for an ileostomy. My concerns are mainly - risks to vulnerable clients if the bag bursts or leaks while i am on one -to - one at the clients house, concerns that they feelworied/burdened by my problems. Risks that I may be distracted by the "bag" while I'm listening and not be as efficient as i should. Embarassement of getting rid of the contents at work as well as leakage whilst trying to be professional in meetings etc. Difficulty in attending to the iliostomy whilst travelling to and from clients and looking for toilets. My own stress and anxiety. I will be dismissed anyway as i have been off sick for a long time - more info to be provided on request.Read more
