Through their services, the specialist teams help patients to manage their heart condition and help them to stay well. The aim of the Cardiac Rehabilitation service is to help people recover and resume a full life as quickly as possible following a heart attack, angioplasty or coronary by-pass and to help people who have heart conditions such as heart failure or angina. Some people think that, because they were in and out of hospital so quickly, their heart attack was not a serious event. But a heart attack is a serious event and cardiac rehabilitation is very important for those people as well. The service offers several rehabilitation programmes that are tailored to suit the individual’s need.

There are hundreds of products that can help you overcome daily challenges and increase your independence. The Podiatry service provides comprehensive, specialist foot care to all patients who are registered with a Wirral GP and meet the agreed referral criteria. The service is provided across ten clinical sites but some treatments are only available from St Catherine’s Podiatry Clinic, Birkenhead. A domiciliary service is provided from all sites for any patients who are medically unfit to be transported to the clinic. The service works closely with Arrowe Park Hospital providing ward rounds and specialist clinical service for orthopaedic and diabetic patients. Referral pathways exist between the hospital and service to ensure a seamless service for patients who require ongoing podiatry treatment.

The service consists of two Specialist Nurses trained in advanced wound care and a registered Wound Care Nurse. Community Nurses, Practice Nurses, Care Home RGNs, GPs and other allied health professionals will contact the service for further specialist advice when their patient has a non-healing or chronic wound. The service will arrange to meet with the healthcare professional and patient where they are normally treated i.e. in the patient’s home, GP practice or clinic to carry out a in-depth specialist assessment. This will include looking at their current treatment(s) and care plans and listening to any concerns the patient has. The service works with patients and healthcare professionals to improve their quality of life through jointly set wound care plan.

End of life care helps all those with advanced, progressive, incurable illness to live as well as possible until they die. It enables the supportive and palliative care needs of both patient and family to be identified and met throughout the last phase of life and into bereavement. The End of Life Care Team was established to ensure that systems and processes are in place as identified within the National End of Life Care Strategy (DH, 2008) to facilitate the delivery of high quality end of life care, irrespective of the patient’s diagnosis. This is done by encouraging the use of the three nationally recognised End of Life Care Tools.

This could be classified as routine dentistry on challenging patients. We also carry out annual dental inspections (dental screening) of school children attending special schools and adult special needs establishments as well as annual epidemiological surveys to monitor trends in dental health and oral health inequalities.

The service provides assessment, diagnosis and treatment for patients with suspected DVT. Anti Coagulation services aimed at preventing blood clots are available for patients with Atrial Fibrillation (a heart condition that causes episodes of irregular and often abnormally fast heart rate). The service is provided at Arrowe Park Hospital or in the patient’s own home if they are housebound. The service accepts referrals from GPs for registered Wirral patients using the DVT referral form or the AF referral form. (NOTE: Only Wirral health professionals are able to access these forms).

TB is a bacterial infection that mainly affects the lungs although it can affect any part of the body. TB is spread through tiny saliva droplets in the air, usually through sneezes and coughs. However, prolonged contact is usually needed for infection to occur. Although many people think TB is a disease of the past, everyday at least one person in the UK dies from TB. TB is treated with special antibiotics. Treatment usually lasts about six months and completing the course of treatment is vital to cure TB and prevent a more resistant form of TB returning.
