Short profile:
The Guildford Community Mental Health Team for older persons offers assessment - rehabilitation - post discharge follow up and monitoring. The aims of the team are to maintain quality of life in the community - prevention of hospitalisation - continuing of patient care - acceleration of rehabilitation and support of families. The service liases with a wide range of other statutory and voluntary health and social care services to provide a well rounded service including day centres - residential homes and the Citizens Advice Bureau.
