Short profile:
At Gateholme we provide support for people with learning disabilities and for individuals who may also have a physical disability. We have a successful record of supporting people to move into more independent environments. At Gateholme we provide support for adults with learning disabilities. We have a number of different houses at the service and each one is personalised by individuals living there. Everyone chooses the colour schemes and furniture for their own rooms and all communal areas. Each house has its own dedicated staff team who provide support that is tailored to each individual's needs. Everyone we support has their own unique Person Centred Plan which they help design. Each plan is designed to support an individual's personal choices and aspirations and focuses on what is needed to make that happen.
Detailed description:
We have a dedicated activities co-ordinator who supports each individual to design their own personalised activities programme. Individuals are encouraged to develop their daily living skills in a range of different areas; such as managing their own money, or looking after their own personal care wherever possible. Activities are varied, ranging from baking and football, through to day trips out in a variety of locations. There is a large garden with a vegetable plot, where many get involved in growing vegetables and also looking after the chickens which have been reared from eggs.
