Short profile:
We care for people of all ages who have community health and mental health and social care needs. We also provide specialist support for people with learning disabilities. We work within the Bradford District and the Craven district of North Yorkshire and offer services in hospitals, the community, and in people's own homes. We are committed to providing a service that is of a high quality, close to where you live and tailored, as much as is possible, to your individual needs.
Detailed description:
As a carer we acknowledge the critical role that you have in supporting the person you care for and we understand the difference this makes to them. We also know that carers may need support themselves. Community Health Champions bring many benefits to their communities by spreading positive health messages in an informal way.
This can be very valuable and many of our Community Health Champions have made a lasting impact on people’s lives in this way. The first step to getting involved is to attend a workshop session centred around ‘Healthy Eating’, ‘Mental Wellbeing’ or ‘Physical Activity’.
So far we’ve had over 1, 000 Community Health Champions doing just this, but we would like to engage with even more people.
