From our growing hospitals we provide a full range of diverse, linked, complex mental health services to the NHS. Following significant investment we are developing new services on both hospital sites providing a whole system pathway of care for patients with mental health problems.
Given that Affinity Healthcare offers linked services on single hospital sites along with access to a broad range of skills within its multidisciplinary teams, we have the ability to effectively treat more complex patients with dual diagnoses.
Detailed description:
Affinity Healthcare provides complex mental health care services to patients who need our services. Care is normally funded via the NHS and patients can enter our services either for a specific treatment or for a series of treatments across our services. The patient is at the core of everything we do and we support them through the Care Programme Approach (CPA).
For people in our care this means:-
One multidisciplinary team providing integrated care and continuity of the therapeutic relationship
Care focused on achieving the best possible outcomes
A sense of success from moving through services towards more independent living
Pro-active patient management rather than reactive support
Highly individualised care programmes
Treatment plans which will be quickly modified if patient circumstances change
An emphasis on self-reliance and a smooth return to independent living
